What is a move-in assessment?
A move-in assessment is the medical and functional evaluation performed at or before admission to an assisted living facility to determine a resident's care needs and establish the initial care plan and pricing.
When someone enters an assisted living community, staff conduct a move-in assessment to understand what level of care and support that person requires. This evaluation typically covers medical history, current medications, mobility and balance, activities of daily living (bathing, dressing, toileting, eating), cognitive status, and any behavioral or safety concerns. The assessment may also document existing medical equipment, dietary restrictions, and emergency contacts.
The assessment results directly inform the initial care plan, which outlines the specific services and level of supervision the facility will provide. This plan becomes the baseline for staffing decisions and daily routines. The assessment findings also determine the resident's placement into a care tier or service level, which establishes the monthly rate charged by the facility. Different levels of care (basic assisted living, higher-level care with more hands-on support) carry different fees.
A thorough move-in assessment matters because it prevents mismatches between a resident's actual needs and the facility's ability to deliver appropriate care. It protects both the resident and the facility by setting clear expectations upfront.
